Sign-up Checklist

1. Complete the application form and sign indicating your agreement with the Articles of Faith of Baptist Mid-Missions.
2. Be certain to give us your pastor's personal email address as we will contact him for his recommendation and church approval.
3. Applicants under 18 years of age: Please include a written paragraph explaining your desire to participate.
4. Include a check for $25.00 written to Baptist Mid-Missions with "¡PlayBall! Account" written on the memo line. This is a non-refundable registration fee to be used entirely for ¡PlayBall! ministries. Participants are responsible for all transportation costs to and from the Dominican Republic and for the following ministry expenses: first-time participants $800.00; veteran participants $700.00
5. Send your registration fee to: ¡PlayBall! ATTN: Sandi Beam, c/o Baptist Mid-Missions, PO Box 308011, Cleveland, OH 44130-8011.
6. Upon confirmation of the approval of your application, the balance of the ministry expenses will be due by June 1, 2026.

Deadline for the application is February 28, 2026.
I hereby declare by my signature that all information given here is accurate and I agree to submit to BMM's Articles of Faith . I am willing to work with a cooperative spirit within the guidelines, procedures, and standards of ¡PlayBall! and its leadership. As a volunteer, I understand that Baptist Mid-Missions and ¡PlayBall! have no liability for my finances, travel, health, insurance, or safety.

I give my permission for Baptist Mid-Missions, its missionaries, staff members, and affiliated subsidiaries to take and make use of photographic images, motion pictures, video footage, and electronic sound recordings of me and my family members, including any minor children of mine, in electronic and/or print media. I also give them permission to use photographs and video footage that I submit, along with my written communication when it is requested, including testimonies and articles, and the transcription of my oral communication given during interviews for articles in Baptist Mid-Missions’ media.

I authorize the use of the above items for any purpose deemed appropriate by Baptist Mid-Missions, including, but not limited to use in its publications, website, social media, and promotions.

I hereby acknowledge the inherent risk of international travel and the fact that injury, disease, or death might occur during or as a result of my voluntary service on a Baptist Mid-Missions mission trip. I fully understand that the risks associated with such service and any related transportation may include, but are not limited to, injury or death by accident; disease; terrorist acts; adverse weather conditions; inadequate medical care; damage to, or loss of, personal property; as well as the unknown dangers and hazards which may arise in the course of my participation on this trip. I realize that should medical treatment be necessary, it may be several hours to days away.

I, in consideration of the benefits derived from being accepted for service, hereby volunteer my services despite such hazards. I willingly assume these risks, and I hereby waive any and all claims against Baptist Mid-Missions and its affiliates (hereinafter BMM), including its personnel, officers, and directors, for any and all causes in connection with the activities of this trip. I, my heirs, or my executors cannot sue BMM, and if I do, I, my heirs, or my executors will be liable for Attorney and Court fees associated with any litigation against BMM.

I have indicated on my application any health condition the team leader should be aware of that may hinder my participation in any activities on this trip. However, I understand that it is solely my responsibility to determine whether I have any medical condition which should limit my participation in specific activities on this trip.

Permission for the above uses shall remain ongoing unless I revoke the permission in writing.
